Here are the likeliest causes:
a. Your nodal point is not correctly aligned: this is the most common cause of ghosting and blurring problems. Make sure your nodal point is correctly lined up in your camera and tripod setup.

b. Objects moved between photographs: objects can move in the time it takes the user to shoot two consecutive images, such as trees and plants in the wind, clouds, cars and people. Shooting as fast as possible helps eliminate these issues, but sometimes they are simply impossible to avoid.
c. Distortion was not correctly calibrated: If a lens produces distorted images and it is not corrected, it will introduce inaccuracies in the rendering. Be sure to use Stitcher’s High Distortion Calibration command to minimize any distortion issues that may cause ghosting and blurring. To calibrate the distortion in the images with Tools - High Distortion – calibrate after dragging two images with a lot of features in common into the working area.
If ghosting and blurring persist, here are a few ways to fix it in general terms:
Use the Stencil tool wherever possible: the Stencil tool helps eliminate ghosted areas by making sure Stitcher selects only one of the overlapping sections of multiple images instead of mixing between many, producing sharp results. It is particularly useful with moving objects and people.
Render your panoramas to PSD layers format: adjust each layer’s blending mask to minimize the blending area where there is ghosting, or to eliminate complex moving objects between images. This is a highly effective technique that works in most cases.
Use the new Enblend feature: Enblend is an external blending algorithm available in Stitcher 5.04 that generates sharp results where needed, and softens up blending where appropriate, all done automatically, usually getting rid of most ghosting and artifacts.
